Coastal Gateway Health Center is asking residents of Jefferson, Chambers, Galveston, and Liberty counties to weigh in on the community conditions and health issues that matter most to them. The health center has launched a Community Health Survey as part of its 2026-27 Health Center Needs Assessment (HCNA). The survey is open from October 1 through December 1, 2026, in both English and Spanish.
Responses will help the health center identify the community’s most pressing health concerns and guide how it plans services and programs in the years ahead.
“No one understands the health needs of our community better than the people who live and work here,” said Kaley Smith, CEO of Coastal Gateway Health Center. “This assessment gives our neighbors a voice in identifying gaps in care and the services that matter most to them. Every response helps ensure that, as Coastal Gateway grows, our programs and services continue to reflect the needs of the communities we serve.”
Residents can take the survey by scanning the QR code below.
The health center encourages people who take the survey to share it with family, friends, neighbors, and coworkers. The more residents who respond, the clearer the picture of the community’s health needs becomes.
